This notice describes how medical information about you may be used and disclosed and how you can get access to it. Please review it carefully.

Your rights

You may ask to inspect or receive a copy of your health information, request a correction, request confidential communications, ask us to limit certain uses or disclosures, request an accounting of certain disclosures, receive a paper copy of this notice, choose a personal representative, and file a complaint if you believe your privacy rights have been violated.

To exercise a right, contact our Privacy Officer. We will respond as required by applicable law. A personal representative may act for you when we verify that person's authority.

Your choices

For certain health information, you may tell us your preferences about sharing information with family, close friends, or others involved in your care or payment for care, and in disaster-relief situations. We will follow your instructions when required. We will not use or disclose your health information for marketing, the sale of your information, or most psychotherapy notes without your written authorization.

How we typically use and disclose health information

We may use or disclose protected health information to provide treatment, coordinate care, operate and improve our services, contact you when needed, and bill or obtain payment for services. We may also use or disclose information when permitted or required by law, including for public-health and safety activities, health oversight, workers' compensation, certain law-enforcement or government requests, legal proceedings, organ donation, medical-examiner or funeral-director duties, and research subject to applicable protections.

Substance-use disorder records

To the extent we maintain substance-use disorder patient records subject to 42 CFR Part 2, we will not use or disclose those records in civil, criminal, administrative, or legislative investigations or proceedings against you without your written consent or a court order and subpoena, except as otherwise permitted by law.

Our responsibilities

We are required by law to maintain the privacy and security of your protected health information, provide this notice, follow the duties and practices described here, and notify you promptly following a breach that may have compromised the privacy or security of your information. We will not use or disclose your health information in a way not described in this notice unless you give us written authorization. You may revoke an authorization in writing, subject to information already used or disclosed in reliance on it.

Changes to this notice

We may change the terms of this notice. Changes will apply to all protected health information we maintain, and the revised notice will be available upon request, at our office, and on this website.
