This notice explains how Lead Healthcare Services collects, uses, protects, and shares personal information submitted through this website. It applies to website visitors, care inquiries, referrals, callback requests, and job applications. We also record limited, first-party website usage information, such as the pages visited, to understand how the site is used. We do not use the public website to collect medical records or detailed clinical information.
Please use public forms safely
Do not submit medical records, diagnoses, insurance numbers, Social Security numbers, financial-account information, or other highly sensitive information through a public website form. If care is arranged, our team will provide appropriate instructions for sharing information through approved channels.
